The mission of Horizon Hospice is to promote comfort and encourage quality of life no matter how long or short that life may be. Hospice will not quicken death nor prolong life. We promote physical, emotional, social, and spiritual well-being to those certified as terminally ill.

What is hospice?

Hospice is end-of-life care for individuals with a terminal illness. Choosing hospice does not mean a patient is giving up on life. Instead, the patient's care focuses on the quality of life and comfort, rather than cure. Neosho Memorial Horizon Hospice provides specially trained staff to assist in care for patients and their families. 

Who is eligible?

Hospice is considered for individuals with a terminal illness whose doctor believes he or she has six months or less to live if the illness runs its natural course. 

It is important to understand that some medical care may continue as long as it supports the goal of comfort and quality of life that remains.

Where is hospice care provided?

Hospice is an approach to care, so it’s not tied to a specific place. It can be offered in settings such as private homes, nursing homes, or assisted living centers. 

Who provides the care?

Our hospice team creates an individualized plan for patients and families based on their needs.

  • Bath Aides
  • Patient's Physician
  • Chaplain volunteers
  • Hospice trained nurses
  • Volunteers
  • Social Worker
  • Bereavement counselor

What does hospice provide?

Hospice provides regularly scheduled nursing visits to assess and treat symptoms. Team members are available 24 hours a day to answer questions or visit anytime the need for support arises. Hospice also provides comfort care medications, durable medical equipment, and other supplies that may be required or deemed necessary for quality care in the home or home setting.

Who pays for hospice?

Hospice is fully covered by Medicare, Medicaid, and many private insurance companies. NMRMC staff can help you check with your insurance company for eligibility. Most plans cover hospice care, medical supplies, medical equipment, and medications related to the hospice diagnosis.

What area does Horizon Hospice serve?

Neosho Memorial's Horizon Hospice serves patients living within a 25-mile radius of Chanute including, parts of Neosho, Allen, Woodson, Wilson and Bourbon counties.

When should I contact hospice?

Often patients and families tell us, “we wish we had called sooner." Early referrals to hospice help patients access and receive the maximum benefit and scope of services. The best way to determine if hospice can help is to contact us. There is no cost or commitment to you or your loved one for calling.

History of NMRMC Hospice

Neosho Memorial established Horizon Hospice in 1992 as a not-for-profit organization. It is our honor to serve the families of Neosho county as well as parts of Allen, Woodson, Wilson and Bourbon counties.

Community support thorough donations, memorials, and bequests are gratefully appreciated. Donations to the Foundation of NMRMC can support Horizon Hospice and promote our charitable mission.All contributions remain in this area.
