Neosho Memorial Regional Medical Center’s case management department helps patients and their families by planning and coordinating hospital and after-hospital care.
Case managers consider the patient’s entire stay. They advocate on behalf of the patient while working directly with hospital departments, physicians, family members and insurance carriers to determine the best course of action while connecting the patient with available high-quality resources.
The department’s four registered nurses and one social worker use their expertise to coordinate services, including those the patient might need after their hospital stay, such as long-term care or home health services. They can also refer people to appropriate community resources.
